Only patients who are already registered in our clinic EMR may request an appointment through this form.

New Patients:

If you are a new patient, please request an appointment only if you have received a notification from the clinic inviting you to book. New patient visits require a referral from a licensed physician or nurse practitioner in Ontario. Once your referral is reviewed and accepted, the clinic will contact you with booking instructions.

Existing Patients

If you are an existing patient, you may use this option to request a follow-up appointment for ongoing care or management of your neurological condition.
