New Patient Experience
At the Neurocentre of Eastern Ontario (NEO), we aim to make your visit as smooth and comfortable as possible. Our team is committed to providing a welcoming, respectful, and patient-centered experience while delivering high-quality neurological care.
Referral Process
Appointments at NEO are available by referral from a family physician, nurse practitioner, or another healthcare provider. Once a referral is received, it is reviewed and triaged to ensure appropriate scheduling. Patients will be contacted by the clinic regarding appointment details.
Appointment Scheduling
NEO uses Appointment Request System (ARS) for appointment scheduling and TELUS Collaborative Health Record (CHR) as the clinic’s electronic medical record system.
Once an appointment is confirmed, patients will receive a confirmation message by text message on your mobile phone and/or by email with the details of their visit.
Intake Questionnaire
Prior to the appointment, patients will receive an intake questionnaire through the TELUS CHR system. This questionnaire helps the clinical team understand the patient’s symptoms, medical history, and current concerns before the visit.
It is important that patients complete the intake questionnaire before their appointment, as this information helps the clinical team prepare for the consultation and improves the efficiency of the visit.
Preparing for Your Visit
To help ensure a productive consultation, patients may be asked to bring the following to their appointment:
A valid health card
A list of current medications
Relevant medical reports or test results if available
Information about previous treatments or investigations related to the neurological concern
Patients may also find it helpful to prepare a brief summary of their symptoms, including when they started and any factors that worsen or improve them.
During Your Visit
NEO uses a collaborative clinical care model. Patients may first be seen by a Physician Assistant (PA) who will conduct a detailed clinical intake, including review of symptoms, medical history, and parts of the neurological examination. This care model allows the clinic to improve access to neurological consultation and reduce wait times, while ensuring that patients receive a comprehensive assessment.
Following the initial assessment, the neurologist will review the findings, perform additional examination as needed, and discuss diagnosis, investigations, and treatment options.
