The Neurocentre of Eastern Ontario (NEO) is committed to protecting the privacy and confidentiality of personal health information. This information may include a patient’s name, contact details, health card number, medical history, medications, test results, consultation notes, and other health information relevant to diagnosis and treatment.

Information is typically collected directly from patients, referring healthcare providers, hospitals, laboratories, pharmacies, or other healthcare professionals involved in the patient’s care.

Use of Personal Health Information: Personal health information is used primarily to provide medical care and support patient treatment. This may include reviewing medical history, ordering and reviewing tests, providing consultation services, coordinating care with other healthcare providers, and maintaining accurate medical records.

  • Information may also be used for administrative purposes such as appointment scheduling, billing, clinic operations, and quality improvement.

Disclosure of Information: NEO may share personal health information with other healthcare providers involved in a patient’s care, such as family physicians, specialists, laboratories, diagnostic facilities, or hospitals, when necessary for treatment or coordination of care.

Protection of Personal Health Information: NEO takes reasonable administrative, technical, and physical safeguards to protect personal health information from loss, theft, unauthorized access, or disclosure. Access to Personal Health Information: Patients have the right to request access to their personal health information and to request corrections if they believe the information is inaccurate or incomplete. Requests for access or correction may be made by contacting the clinic. Certain limited fees may apply for copies of medical records in accordance with applicable regulations.
